You do not have to give a reason why you are applying for your notes.

The 1997 Freedom of Information Act, allows women and their partners to seek their medical notes. You can make an application through the patient liaison officer in the maternity hospital where you gave birth. Some basic information is required.

  • Name.

  • DOB

  • Baby’s name and DOB

  • Address

  • Contact phone number

  • Photocopy of your ID

  • Name of care provider

There is also a Freedom of Information Act (FOI) form that you can complete online. An FOI officer will then contact you. Your notes are then delivered free of charge to your home by registered post or a specific time can be arranged for you to collect them from the hospital.

Template For Writing Preferences for Labour & Birth

Writing your birth preferences is a really effective way of communicating your preferences for birth with your care provider. Ask your doctor / mid-wife if they can review it and sign it at your next antenatal appointment. Also ensure that your birth partner is aware of your wishes for birth. Which means on the big day you don’t have to explain anything!
